Idk why everyone makes it sound complicated. Just go to Vault 81, save a cat, then wait for a quest trigger. Easy peasy.

To kick off 'Hole in the Wall' in Fallout 4, you gotta head to Vault 81 first. Make sure you're carrying either 3 Fusion Cores to hand over or have enough Charisma to sweet-talk your way in. Once inside, mingle and do a few chores until a quest called 'Here Kitty, Kitty' pops up. Help out by finding Ashes the cat and bringing him back. After that's done, wait or wander around the vault until you hear about a medical emergency over the intercom - that's your cue that 'Hole in the Wall' is ready to start.
