I've been playing for years, and the only time my dwellers got sick was when I forgot about my water treatment facilities. It's all about that clean water supply and having enough RadAway. Outfits with endurance boosts help too, since they increase dwellers' resistance to radiation.

Easy peasy, radroach pie! Just kidding, but seriously, keeping your Dwellers' water levels above the required threshold is key to stopping radiation poisoning. If the water supply dips below the line, your Vault residents start absorbing radiation - no good. Keep those water purification rooms running smooth and upgrade them as soon as you can. Also, make sure to equip your Dwellers with outfits that boost their Strength, as it helps in water production. And don't forget to use RadAway! It's specifically designed to cure radiation sickness. Keep a good stock of it by either producing it in the Vault or snagging some during your Wasteland explorations.
