I've done this a bunch of times – get a capture card, link up your PS4 to your PC, and stream via OBS. Super smooth setup once you get it going!

You'll need to use a capture card like Elgato HD60. Connect your PS4 to the capture card and then to your PC. Use OBS Studio to stream the gameplay, and then share that OBS window in your Discord video call. Boom, done!
