To summon the Eye of Cthulhu in Terraria, you gotta grab yourself a Suspicious Looking Eye, which you can craft at a Demon or Crimson Altar using 6 Lenses from those pesky Demon Eyes that fly around at night. Alternatively, if you're lucky, you might find one in a chest. Once you have it, wait for nighttime (begins after 7:30 PM in-game time) and then use the item in your inventory. Just make sure you're ready for a fight, because this boss doesn't play around.

Hot take: Just wait around at night, especially if you've got a decent amount of health and some NPCs moved in. The Eye of Cthulhu might just surprise you and show up without an invite. No crafting needed, but definitely not for the faint of heart.

Actually, if you want a real challenge, try summoning the Eye of Cthulhu without any armor. Did that once by accident because I didn't know what the Suspicious Looking Eye was for. Talk about a surprise party at night!
