Just signed into my new phone yesterday. All my Pokémon, items, and progress showed up exactly as I left them. Make sure to reconnect your Google or Facebook account though!

Make sure you know your login info (email and password) for Pokémon GO. Download the app on your new phone and sign in. Your Pokémon should transfer over no problem!
