Make sure you're signed into the same Game Center (iOS) or Google Play (Android) account. Then, follow these steps: Go to the 'Settings' menu in the game on your old device, and select 'Cloud Save'. On your new device, open Sims FreePlay and go to the 'Settings' menu again to select 'Cloud Load'. Voilà! Your game should transfer over.

Just log into the new device using the same Facebook or Game Center account linked to your game. Easy peasy!

You might not need all this fuss with logging in and out. I've done it before where just using Facebook syncs everything automatically. Worth a try!
