Super easy! Just plug your headset into the DualShock 4 controller. Your PS4 should auto-detect it. If not, go to Settings > Devices > Audio Devices and select your headset.

Try this: Settings > Devices > Audio Devices > Input Device, then select your microphone. Sometimes you need to adjust the mic levels there too.
