First, you need to press the Options button to open the pause menu. Then, press L1 + R1 + Square + Triangle simultaneously. This will enable creative mode.

Fire up your map, hit the Options button, then press L1 + R1 + Square + Triangle at the same time. You've now unlocked creative mode!
