Beating Mom's Heart a few times unlocks the alternate path, including Downpour. Easy-peasy!
To unlock Downpour, you need to have access to the Mirror Room. Start a run, get to the Mines or Ashpit, and find the Minecart Rail. Ride it, and you'll find the Mirror Room on the other side!
Uses bombs wisely. Bomb blue rocks to access hidden rooms that sometimes connect to the Downpour path. Good luck!
