You can unlock the Goat Head by defeating The Sin boss in The Basement, or you can find it randomly in golden chests and devil rooms. It's RNG, so good luck!

Best way to get Goat Head is luck. Sometimes it just appears in devil rooms or chests. Keep opening them!
