To unlock the True Pacifist ending in Undertale, first, you need to complete a Neutral run. Then, reload your save file and go on a date with Papyrus and Undyne, help Alphys with her issues, and make sure no one dies during your playthrough!

Just don't kill anyone and make friends with Papyrus, Undyne, and Alphys. Easy peasy!
Firsthand tip: I did it by completing the Neutral route, then befriending Papyrus, Undyne, and Alphys. It was totally worth it!
Complete the neutral ending first. Then, reload the game, befriend Papyrus, Undyne, and Alphys, and keep everyone alive for the True Pacifist ending!
