Just plug an HDMI cable from the 360 to the capture card, then another HDMI to the TV. Simple!

I hooked my Xbox 360 to my Elgato HD60S by connecting the HDMI cable from the Xbox to the input of the capture card, then another HDMI from the output to my TV. Worked like a charm!
