In a game, tap the player on base, hit 'Substitute' and pick your pinch runner. Did this in my last game and it totally worked!

Easy peasy! During a game, just tap on the player you want to replace on base, then pick 'Substitute' and select your speedster. Boom, pinch runner activated!
