To use an Upgrade in Pokemon GO, select the Porygon you want to evolve, tap on 'Evolve', and then you'll see the option to use Upgrade along with 25 Porygon Candy!

Honestly, just use the Upgrade on Porygon like you use any other evolution item. Tap evolve and let the magic happen.
