Had to do this before selling mine! Make sure to deactivate your PSN account under Settings > Account Management > Activate as Your Primary PS4 and select 'Deactivate.' Then go through with the full initialization.

To wipe your PS4 before selling, first, back up any data you want to keep. Then go to Settings > Initialization > Initialize PS4, and choose Full. This will erase everything!

Don't forget to remove any discs from the drive! Then follow Settings > Initialization > Initialize PS4 and pick the full option. Done deal!

Quick tip: if your PS4 is acting up and you can't access settings, you can boot into Safe Mode by holding the power button till you hear two beeps. From there, you can choose Initialize PS4.
