Unfortunately, no. Surgeon Simulator 2 does not support split screen play on Xbox.

Sadly, Surgeon Simulator 2 on Xbox is not split screen. Time to find a new co-op game!

I've tried it myself, and Surgeon Simulator 2 doesn't offer split screen on the Xbox. Just online multiplayer.
