I picked Toughness in my first playthrough, and honestly, I felt like it made a difference in early game survival. Having that extra damage resistance let me explore more freely without constantly worrying about getting one-shotted by raiders or ghouls.

I say skip it. Fallout 4 is all about flexibility, and there are way better perks that give you more bang for your buck early on. You can always find or craft armor that gives you the resistance you need without wasting precious perk points on Toughness.
