A save in MLB The Show 23 counts when a pitcher finishes the game for the winning team under certain conditions. These include: entering with a lead of no more than three runs and pitching at least one inning, entering with the potential tying run on base, at bat, or on deck, or pitching three innings to finish the game.

If your reliever keeps the lead and closes the game, that's a save!

To get a save, the pitcher must be the last one to pitch and meet one of the following: the lead is three runs or less, they pitch for at least three innings, or they enter with the tying run on base, at bat, or on deck!

From my experience playing MLB The Show 23, your reliever needs to finish the game with a lead of 3 or less or handle a pressure scenario (tying run on base, for example).
