When you hit the Stimpak cap in Fallout Shelter, any additional Stimpaks you try to produce or collect from the wasteland just vanish into the ether. Your med bays won’t produce any more until you dip below the limit again. Moral of the story: Use them or lose them!

If you're at Stimpak capacity, you can't hoard any more. It's basically the game telling you to stop being a Stimpak miser and start using them to explore the wasteland or heal your Dwellers. So, get out there and start adventuring!
Honestly, reaching Stimpak capacity just means you're doing great at managing your resources. Consider it a pat on the back from the game.
Nothing much, you just can't make or pick up more till you use some. It's kinda annoying tbh.
