A save in MLB The Show 23 happens when a relief pitcher finishes a game for the winning team under certain conditions. These include entering the game with a lead of no more than three runs and pitching at least one inning, or entering the game with the potential tying run either on base, at bat, or on deck.

From my experience, a save is earned when your closer shuts the door on the opposing team in late innings of a close game. Nothing more satisfying!

In MLB The Show 23, a pitcher gets a save if he enters in a save situation and maintains the lead until the end of the game. Simple as that.
