In Tokyo Ghoul, the eye transformation is when a ghoul's eye turns black with a red iris. It's called a Kakugan. Pretty cool, right?

Kaneki and other ghouls get a gnarly eye makeover. One of their eyes turns all black and red, which means they're in ghoul mode.

Basically, when a ghoul activates their powers, one or both of their eyes change. It's kinda like their superpower indicator.
