Honestly, it's M for Mature. Makes sense, what with all the mutated creatures and the post-nuclear war setting. It's not exactly a tea party.

Depends on who you ask. Critics were tough on it at launch, but if you're asking about the ESRB rating, it's Mature. Players have seen a lot of improvements since release, so don't let the early reviews fool you.
