Fullmetal Alchemist (2003) diverges from the manga with an original story, while Brotherhood (2009) sticks closely to the manga. Both have unique endings and plot points!

2003’s Fullmetal Alchemist starts the same but veers off into an original plot halfway through. Brotherhood, though, follows the manga from start to finish. Simple as that!

From my experience, Brotherhood provides a more faithful adaptation of the manga with deeper character development and a more coherent storyline.

Short and sweet: FMA 2003 has a different ending and unique subplots, Brotherhood sticks to the manga. Both are awesome for different reasons!
