Worst case scenario, you might need a new battery or a new controller. It happens!
Reset the controller. There's a small reset button on the back of the controller near the L2 button; press it with a paperclip.
Make sure the charging port on the controller is clean. Dust and debris can easily get in there and mess with the connection.
