The first plague heart in State of Decay 2 is always random, so it could be near your starting base or a bit further out. Just follow the red pulsing on the map, and you'll find those nasty hearts!

In my experience, the first plague heart is often located in one of the buildings close to your starting location. Keep your eyes peeled for creepy red goo!
