Go to the Base of Operations at the White House and find the Barber. He'll take care of all your makeover needs!
You can change your appearance at the Barber in the White House. If you can't find it, just look for a pair of scissors on the map.
Tbh, changing your character's appearance mid-game is overrated. Just focus on gear and skills!
