From my experience, start by accessing the DLC content through the in-game main menu, then warp to new locations from the Teleport Gate Plaza.

You can do the DLC stuff in Sword Art Online: Hollow Realization by heading to the Additional Content areas in the main menu. Check out the Teleport Gate Plaza in town for new missions!
