In the Steam app, bottom left corner, there's a '+ Add a Game' button. Click it and choose 'Activate a Product on Steam...' That's where you input your code. Easy peasy.

Pro tip: make sure your code hasn't expired! Some of them do, and you don't wanna waste your time.

Just head over to the Steam website and log into your account. There's an option under your profile name called 'Redeem a Steam Wallet Code'. Can't miss it!
