Dmitri Mendeleev created the first modern Periodic Table in 1869. He left gaps for undiscovered elements, which he predicted correctly!
Mendeleev, hands down. He arranged it in a way that made sense, leaving blanks for elements not yet discovered.
Some say Dmitri Mendeleev, but John Newlands and Lothar Meyer also made significant contributions. Mendeleev just got the fame!
