If you can't revive your dweller in Fallout Shelter, it's likely because they died during a quest or in the Wasteland. Dwellers that die inside your Vault can be revived with Caps, but if they perish on a quest or while exploring, you need to recall them to your Vault first. Once they're back, you can revive them with Caps. Always make sure you have enough Caps on hand for these emergencies!
Sometimes it's just a glitch. Try restarting the game or checking for an update. If nothing works, contacting support might be your best bet. But 9 out of 10 times, it's usually because of not having enough Caps or not recalling your Dwellers first.
