Honestly, it's probably because Xbox One is getting a bit old and Fallout 4 is a beast of a game. I upgraded to an Xbox Series X and the difference is night and day. No more freezes.

It's all about the mods! They're great but can mess things up. Try running the game without any mods and see if you still get freezes.
