Your PS4 might be in Safe Mode because of a failed system update or corrupted data. Try restarting it or updating the system software using a USB drive.

Happened to me when my power went out during a game. Make sure your cables are properly connected and your power source is stable.

It's likely because of a corrupted firmware or software issue. Rebuild your database from the Safe Mode menu to fix it.
