Mortal Kombat is too violent for Japanese taste, plain and simple.

The game's ultra-violent Fatalities don't sit well with Japanese censorship laws, so it's not officially released there.

Having spent some time in Japan, I can say that their gaming culture favors less graphic violence and more fantasy or RPG elements. That's probably why Mortal Kombat isn't a big hit there.
