Check if your PS4 controller is set to work as Player 1 in the settings. If another controller or keyboard has taken that spot, you might run into issues.
Hot take: Make sure you're not trying to use the controller while it's charging via a low-quality cable. Some cables only support charging and not data transfer!
I've had this issue before, and it turned out to be a compatibility problem with the Steam settings. Go to the Big Picture mode, then settings, and enable PS4 Configuration Support.
