To get 12 hearts with someone in Stardew Valley, you need to max out your relationship with them. Start by talking to them every day, giving them their favorite gifts (twice a week and on their birthday), and completing any special quests they offer. Once you reach 10 hearts, you'll need to give them a Bouquet to signal you're interested in a romantic relationship. Keep increasing your bond, and eventually, you'll have the option to give them a Mermaid's Pendant to propose marriage. After marriage, keep showering them with love and gifts to reach those 12 hearts!

Actually, getting to 12 hearts is overrated. 10 hearts is where it's at unless you're aiming for marriage. Then yeah, do all the extra stuff.
