Just hit F to open the build menu, select the hearth, and plop it down on a solid floor or ground. Make sure your roof’s high enough, or you'll smoke yourself out!

First, make sure you have stone, wood, and a hoe. Level the ground, build a stone floor, then position the hearth on top—easy peasy!

I find it's easiest to place a hearth by leveling the ground with a hoe first to avoid any alignment issues.
