First off, you gotta have a fishing rod that can actually use bait, which means either the Fiberglass Rod or the Iridium Rod. You can't attach bait to the basic Bamboo Pole. So, once you've got the right rod, open your inventory, select the bait, and then press 'A' to attach it to the rod. Just make sure you've got the rod equipped when you do this, or you'll just be standing there looking silly.

Super simple: Get the right rod (Fiberglass or Iridium), open inventory, pick the bait, then slap that bait on the rod with the 'A' button. Happy fishing!
