First off, you gotta be friends with Emily, so make sure you're at least at a two-heart friendship level with her. Once you're in, head to her house which is next to Haley's. Inside, you'll find the sewing machine. Just bring a cloth (you can get this from wool using a loom or buy it at the Stardew Valley Fair) and any item you wanna use as a dye or pattern. Pop the cloth into the sewing machine, add your item, and boom, you've got yourself a custom piece of clothing!

Literally just chuck a piece of cloth and another item into the sewing machine at Emily's. It's not rocket science. Make sure the other item has a color, otherwise, you'll just waste your materials.

Fun fact: I tried using a rainbow shell with the cloth, and it made this really cool shirt! Experiment with different items 'cause you never know what awesome outfit you'll end up crafting.
