It's just a way to fix the sync between the game's music and the obstacles. If it feels off, you adjust the offset. Easy peasy.

Honestly, don't mess with it unless you really need to. I tried adjusting my music offset and it threw off my rhythm more than it helped. Just get used to the game's timing as is.
