The Skull Key unlocks the Skull Cavern in the desert. Once you've got it, just head to the desert, find that big door in the northwest corner, and you're in! It's tougher than the mines, so pack plenty of food and bombs.

Honestly, the Skull Key's real value is in showing off how far you've gotten. Sure, it opens the Skull Cavern, but it's also a badge of honor. Like, 'Yeah, I've made it this far.'
